Question: What should I bring to the Initial Appointment? Answer: Completed customer information form - Doctor's orders (Prescription)Insurance cards, old braces(orthotics) or prostheses. Question: Can you just get all of my paperwork for me? Because the existence of paperwork is directly related to a visit by you, it is easier for you to obtain the paperwork. No, your physician must let us know what it is that they would like us to do for you. Question: Does my insurance cover this procedure? Answer: Each insurance policy is different. We will perform a Bio mechanical Evaluation and submit the "codes" to the Billing Department, who then will follow-up individually to discuss the findings of coverage availability. Question: I have no health insurance. Answer: No problem...Call us and we will assist you in anyway we can. Question: What is the "Life Expectancy" of the Orthosis or Prostheses?
